Memorandum of Understanding - Lifeline Education

The following Memorandum of Understanding shall be between Lifeline and the above school district. Lifeline provides sexual risk avoidance education services to over fifty schools across Missouri and Iowa. With over ten years experience, the program continues to provide dynamic results. Lifeline provides all required sexual abuse prevention training in accordance with the DESE requirements in RSMo §170.045. The purpose of this agreement is to commit that the aforementioned school will host the program for grades 5th-12th.

Lifeline Responsibilities

As such, Lifeline shall: 

• Coordinate and schedule presentations with the school 
• Provide a certified educator to lead all class sessions 
• Provide all required handouts and information 
• Conduct program evaluation and provide access to evaluation information as requested 
• Provide all services of the program free of charge 
• Provide parent opt-out forms at least two weeks prior to student class sessions 
• Provide student recruitment materials 
• Provide parent and teacher educational resources to promote abuse prevention

School District Responsibilities

In return, the aforementioned school shall:

• Schedule appropriate dates to host the student program for 5th-12th grade students 
• Coordinate dates for a parent informational webinar and staff professional development session 
• Ensure opt-out forms are sent home at least one week prior to the student program start date 
• Encourage students to complete the take-home activities of the program 
• Provide an appropriate classroom/location for the programs to be conducted and a school representative present in all sessions
• Allow students to participate in a program survey with results published anonymously
